Getting Your Test Results
FPA has an automated system via Televox for you to more easily
access your test results. This system permits you to access test
results via phone or the web. You will be given a LabCall Reminder
card at the office with the appropriate phone and web address.
Obtaining
Results via the Televox Phone System: Your test results will
be available by the date designated to you by the FPA medical
assistant. You may call in at any time, if your test results are
not available yet, the system will notify you. To obtain your
test results via phone, please use the following information as
prompted by the phone message system:
- Dial
1-866-434-3081
- Enter
your Unique ID (your 9 digit social security number-- i.e. 123456789)
followed by the # sign
- Enter
your PIN (your 6 digit date of birth ie. 010158) followed by
the # sign. You may change the PIN at any time.
If
the system does not recognize you, please call FPA to verify that
we have the correct social security and date of birth information
entered for you.
Obtaining
Results via the Televox Website System: As with phone results
above, test results will be posted around the date designated
to you by the FPA medical assistant. You may access the test result
site at any time, if your test results are not available yet,
the web site will notify you. Yo
will also be able to review a history of test results
via this website. To obtain your test results via the internet,
please use the following information:
- Go
to www.mytestresults.com
- Enter
the FPA customer number: 147024
- Enter
your Unique ID (your 9 digit social security number-- i.e. 123456789)
- Enter
your PIN (your 6 digit date of birth ie. 010158). You may change
the PIN at any time.
